Description:
1994 San Marino GP qualifying session, Roland Ratzenberger's Simtek goes off the track, but it seems OK. The driver starts another lap, pushing hard. What he does not know is that the front wing became damaged. It finally breaks off and gets stuck under the front wheels. Being unable to turn, Ratzenberger hits a concrete wall at 315kph and dies on the spot from skull base fracture. 24 hours later, several hundred meters from the same spot, Ayrton Senna crashes…

And the day before that, Rubens Barrichello had gotten airborne over a curb and barely avoided tragedy himself...

Barrichello crash: http://www.youtube.com/watch?v=la0vQlYOKbs

Senna was very upset by Ratzenberger's death and some stories said he wanted the race called off. Unfortunately for all of us, the race went on.

That was my first real shared experience on the web. I was on Compuserve back then and when I heard about the death, I got on their racing chat boards where all of us shared our grief.

The only good thing about that terrible weekend is that both cars and tracks became safer as a result.

#1, There's more, the irony doesn't end there. Senna was agreed to become one of the directors of the reformed Grand Prix Drivers' Association, the aim of which was to increase the safety. Also, after his crash the marshals found a furled Austrian flag in the car, apparently Senna wanted to wave the flag after the race to honor Ratzenberger.

Wasn't there also a major accident in the pits during this race that injured a few mechanics?

That was Jos Verstappen at Hockenheim.

The black weekend in Formula 1. Those three crashes weren't the end of it for the weekend. During one of the pitstops, one of the tyres was not properly secured onto the car and it flew off hitting several mechanics.

I was inspired to post this by my master's that I'm writing (uh, trying to). The funny thing is, that just when you think the safety standards can't get any higher something happens and it becomes clear they're not.
But if there is such a thing as meaningful death, Senna's and Ratzenberger's were it. FIA founded a few safety comitees and projects and in 2004 FIA Safety Institue was founded. HANS finally became popular, even though the device dates back to early 80s and the death of one Patrick Jaquemart. He crashed his LeCar during testing and died from basal skull fracture. The irony is that a lot of well known drivers died the same way, while the device was virtually unknown and drivers refused to help with development as they saw it as 'uncomfortable'.
