I think this is a case of people double- and triple-clicking Submit because they're too damn impatient to wait for the slow-ass server to finish posting their comments.

I don't think I've double-posted or more in years... I learned not to hit the button more than once or to hit refresh when submitting a comment. If I think the site's gonna crap-out, I simply ctrl+c the text I've typed, and wait for the site to be working again, before trying to submit the comment again.

On Firefox if the comment doesn't post because the site times out, just hit back, then forward, and if the page loads properly your comment will show up. Works for me.
